Option 1: Request via Secure Text Link

The Medical Records Department can send a secure text message to your mobile phone.

1. Click the secure link
2. Follow the prompts to complete your request
3. Enter our fax number (833-455-6137)
4. Upload a photo of your ID
5. Submit your request

You will receive an email confirmation once your request has been submitted. Requests are typically processed within 2–5 business days after confirmation.

Option 2: Patient Portal

Patients may log in to their Advent Health Patient Portal to view or print available medical records.

Option 3: Request Medical Records In Person

Visit the Advent Health Hinsdale Health Information Management (HIM) Office to complete and sign a Release of Information (ROI) form.

Option 4: Submit an Online Request Through the Advent Health Medical Records Portal

If you prefer to submit your request online, follow these steps:

Step 1: Visit the Advent Health Medical Records Portal
Go to: https://www.uchicagomedicineadventhealth.org/medical-records

Step 2: Complete the Online Request Form
Fill out the Release of Information (ROI) request form on the portal.

Step 3: Include Our Office Contact Information
To have your records sent directly to us and to avoid being billed for the records. You must include our contact information in the request. Please provide the following:

Step 4: Submit Your Request
