If you are a new patient transferring from Advent Health, please follow the steps below to request that your medical records be sent directly to our office.

Step 1: Visit the Advent Health Medical Records Portal
Go to: https://www.uchicagomedicineadventhealth.org/medical-records

Step 2: Complete the Online Request Form
Fill out the Release of Information (ROI) request form on the portal.

Step 3: Include Our Office Contact Information
To have your records sent directly to us and to avoid being billed for the records. You must include our contact information in the request. Please provide the following:
